  • How do I set an appointment?
    We welcome new and returning patients to our practice! Please call us at 951.788.0858 to schedule an appointment with us. When calling, we need some information including the following: • what type of insurance you have (we have a list of accepted insurance plans) • who is the primary holder • date of birth and social security or ID number of the primary account holder patient's date of birth • patient's phone number to confirm their scheduled appointment
  • Which insurance plans do you accept?
    Most dental insurance plans are accepted at our office. Listed are some of the insurance companies we accept. If you do not see your insurance provider listed, or are unsure of your coverage, do not hesitate to call us. Our staff will verify and explain your benefits before treatment, at no charge. We are unable to accept HMO plans at this time.
  • What services do you offer?

  • I'm a new patient!
    We are excited to have you come into our office! Bring in your printed and completed forms to expedite your office visit. You can download the form above. Your privacy is important to us. All information received in the above forms and through other communications is subject to our Patient Privacy Policy.
  • What should I expect for my first visit?
    We can't wait to see you! You will need to fill out our required forms either at the office or ahead of time. These forms help you tell us some information about yourself and your medical and dental history. We will then bring you back to one of our operatories and begin with one of our assistants taking some x-rays and photos of your teeth. If you have a specific concern, we will always listen and address those concerns. One of our doctors will then work with you to create a personalized treatment plan to meet and manage your specific dental needs, no matter how simple or complex. We take pride in giving our patients all the options of their treatment and all of the benefits and risks of each treatment so that you can make an informed decision. Nothing will be done in our office without your consent. Our office participates in many insurance programs. To find out if we accept your insurance plan, you can call our office at 951.788.0858.
